I realize that there are dangers inherent in glass working, and other studio activities, and that mortal or serious personal injuries and property damage, including (but not limited to) physical effort, cuts, burns, inhalation of hazardous substances, and/or exposure to visible and invisible radiation, may occur from my participation in such activities.

Should any injuries occur during or as a result of participation in any Glass Mermaid Studio class or workshop, I agree to indemnify and hold harmless Glass Mermaid Studio, employees, instructors, volunteers and others associated with the Glass Mermaid Studio.

I give Glass Mermaid Studio permission to seek medical attention for me in case of accident or emergency.

I expressly agree that this waiver and release agreement is intended to be as broad and inclusive as permitted by the Laws of the State of Maine and of any other state wherein such activities may occur, and that if any portion hereof is held invalid, the remainder hereof shall continue in full force and effect.

I agree to allow myself and my artwork to be photographed and grant Glass Mermaid Studio use of these images for promotional purposes.
